Our GPs are committed to helping older adults maintain their independence, health, and quality of life through personalised, compassionate, and comprehensive care. We provide ongoing care in the clinic, Residential Aged Care Facilities, and home visits where clinically appropriate, ensuring continuity of care wherever our patients live.
Our practice also has a full-time Care Coordinator dedicated to supporting patients with chronic disease management and geriatric care. Working closely with our GPs, she helps coordinate care across primary care, Sale Hospital, specialist and tertiary health services, allied health providers, community organisations, and aged care agencies. She also assists patients and families in navigating the healthcare system, ensuring smoother transitions between hospital and home, facilitating referrals, and helping access appropriate community supports.
