Our full-time Care Coordinators work closely with our GPs, nurses, allied health professionals, specialists, hospitals, and community organisations to support patients with chronic illnesses, complex medical needs, and age-related health concerns. By improving communication and coordination across the healthcare system, we help patients access the right care at the right time.

Our Care Coordinator plays an important role in supporting older patients to remain healthy, independent, and well connected with appropriate services. Working alongside our GPs, they assist with comprehensive health assessments, coordination of geriatric care, referrals to community services, and communication between families, residential aged care facilities, hospitals, and specialists.
